Is my dermal rejecting.?

So I had it done a few months ago and after about a month or so the diamond fell out so my piercer reply the top. And for the longest time the top continued to fall out but I would always put or back on knowing it wasn't tighten well. But it fell out for the last time about a week ago and I had to buy a new top. Now I didn't want to keep losing it in the shower nor did I want to go through the pain of having my piercer tighten it so I bought a clap and needle nose tweezers and put the new top on REALLY tight. But I noticed today that I could feel a little bulge from the anchor under my skin and when I went to press on it puss came out. I cleaned and and all that and then I noticed that my star (the top) was a little lopsided. I look all around the dermal and I'm not able to see the post so I guess I'm saying it sits flush with my skin. So is this just an infection or rejection. Its summer so I have been using sunblock and in the shower I use a loofah and it tubs on it sometime. Could these cause an infection. Tomorrow is Sunday so my piercer is closed so I just want to know if I should prepare for to to reject and start treating the skin around it to minimize scaring.
